Mail or deliver the form and check to the address listed below. The school will send a receipt for your tax records.
Qualified activities may include fine arts, extracurricular field trips, community school classes, athletics, and before- and after-school clubs. The amount contributed—up to the applicable limit—may reduce your Arizona tax liability or increase your refund.
